Accident/Hazard Reporting

Prior to undertaking work, employers and employees must identify any safety or environmental hazards that they find.

Once identified, all appropriate personnel need to be made aware of the hazards so they can be controlled.

As the work environment is constantly changing, it is important that any new hazards are reported.

In the case of any safety or environmental incident and/or injury or near miss, management must be notified as soon as possible (within 24 hours).

Depending on the seriousness of the incident, further reporting to WorkSafe or the EPA may be required, so quick reporting is essential.

Many serious incidents (see Fig’s 45a and 45b) may also require the site to be preserved, and work should not continue until your supervisor tells you it is safe/appropriate to do so.

Reporting, investigating and taking action for things that have not gone to plan is important to reduce the chance of an incident happening again.
